You may send a letter to the Town Clerk’s office, 488 Chapin Street, Ludlow, MA 01056, or use one of the following forms:
 
Birth Certificate Request Form
 
Death Certificate Request Form
 
Marriage Certificate Request Form
 
The cost is $10.00/certified copy.
Make check payable to the Town of Ludlow.
Please include a self-addressed stamped envelope.
 
NOTE: Before mailing in your request, please contact the Town Clerk’s office to be certain we have this record. Some records are restricted or impounded and access may be denied. To prevent any delays, please enclose a copy of a government issued ID with your request.
 
Marriage Intentions

How to apply for a marriage intention?

The ceremony must be performed in a city or town within Massachusetts.

 
Both parties will need to come into the Town Clerk’s office, 488 Chapin Street, to complete the "Notice of Intention of Marriage."
 
Please have a government issued ID available.
 
The cost is $15.00 (cash or check).
